Approx 10 days ago I tried to activate a new sim and port a number over from Telstra, but it still has not been completed.
Is there some issues going on with Optus or is a dud sim a possibility?

You might want check the details you provided Optus. If those details you gave Optus do not match the details in the account from Telstra, that might be the reason.
