It asks to set up a my optus account (he has never had one) but when I do they say it is not eligible why????
I’m assuming you say “not eligible” when you try to setup the online account?
you can chat/call to replace the SIM.

It's possible that he's already been set up with a My Account (depending on the age of the service).
I'd try the, 'forgotten your username or password" option.
