Transfer of Service Ownership


I'm trying to complete a transfer of ownership form to pass my internet and home phone onto another person, however every time I enter my details (which I have confirmed are correct through my bills and account information), it says my details are incorrect?

Has anyone else had this problem? I tried to use the Optus App to ask for assistance but they responded 18 hours later and I got no notification despite having notifications turned on, and they just closed my ticket on me when I didn't respond, despite it being 6am and me not being awake..
