Setting up direct debit

Does anyone have any tips for setting up direct debit with Optus? Since receiving several $15 late fees ive tried to set it up 3 or 4 times online but it hasnt worked each time no matter what....ive even gone back to basics and have printed off the direct debit request form and mailed it in and months later im still getting paper bills and late fees because often the bills get lost before we receive them....extremely frustrating, amd getting very sick of wasting $15 per every few months for nothing. Thanks

You might be best to have a word with accounts if the direct debit system keeps failing you. You can do that via the live chat on the second link if you wish.
