I am looking for advice on how to have the 'entertainment bundle' charges permanently removed from my bill.
I have have needed to call Optus for the January, February and March bills to have these charges removed. I've just received the April bill - and the charges are on their again, along with the credit from last month!!!!!!
I assume next next step will need to be the telecommunications ombudsman?

I would make a complaint via the online form below and mention that your next step will be the TIO if the same thing happens again.
