Direct debit form




I want to change my direct debit for a post paid mobile and there has been a technical issue with myoptus for the last few days stopping me from doing so. The website indicates there is a form you can fill in to have this actioned instead.


Where is this form or how can I email optus to obtain it (what is the email)?
